The 14 communities include Providence, R.I.; Upper Capitol Region, N.Y.; Western Pennsylvania; Southwestern New Jersey; Metro Atlanta East, Ga.; Miami; Tuscaloosa, Ala.; Evansville, Ind.; Greater Lansing Area, Mich.; Omaha, Neb.; Baton Rouge, La.; North West Denver, Colo.; Harlingen, Texas; and Whatcom County, Wash.
The Care Transitions Project aims to improve coordination across the continuum of care. The selected communities will work with their states’ Quality Improvement Organizations to implement projects that improve processes that promote seamless transition from the hospital to the home or other facilities.
According to CMS, one in five patients who leave the hospital will be readmitted within the next month, and more than three-quarters of these readmissions are potentially preventable.
