The CAC initiative is similar to an accountable care organization and will strive to meet the triple aim of improving health and patient experience while lowering cost.
Novant Health-employed nurses will serve as care coordinators in the program, helping guide patients through the healthcare system and managing their chronic conditions. The care coordinators will collaborate with Cigna case managers and use patient-specific data provided by Cigna. Additionally, providers in the CAC can be rewarded if they meet certain, unspecified quality and cost targets.
The CAC will be Cigna’s largest in the nation, covering more than 60,000 members.
