The organizations made the announcement Tuesday, expanding Presbyterian’s value-based care initiatives. The eight-hospital system already has about two-thirds of revenue tied to value-based care. The new ACO will add more than 12,300 UnitedHealthcare members with employer-sponsored health plans in New Mexico to Presbyterian’s value-based arrangements.
Under the arrangement, UnitedHealthcare will provide Presbyterian with additional data to support population health management, provide preventive care and lower costs. Patient navigators will also help provide community-based care coordination as part of the ACO.
Presbyterian providers will have the opportunity earn incentives based on patient satisfaction, quality and cost savings.
