The agreement will bring the accountable care model to Humana beneficiaries and HealthSpan Physicians patients in major metropolitan areas in Ohio, including Cleveland, Cincinnati, Youngstown and Toledo. The model emphasizes quality outcomes and coordinated care led by a primary care physician, according to a statement.
The organizations will implement predictive analytics, chronic disease management and wellness programs to help integrate care and promote health.
“Healthcare is like a symphony. You can have world-class strings, brass, woodwinds and percussion, but they won’t sound good together without a conductor,” Humana Ohio Senior Products President Larry Costello said in a statement. “Working with HealthSpan, we’ve put the health care baton back in the hands of primary care physicians and the result will be a better health care experience for our members.”
