The program covers more than 330,000 patients at more than 450 office locations.
As part of the PCMH Program, practices can receive outcomes-based or shared-savings payments if they meet cost, patient experience and patient outcome goals.
“Our patient-centered initiatives are designed to transform how healthcare is delivered in New Jersey with a focus on providing incentives for more coordinated care that results in better patient outcomes and satisfaction at a lower cost,” Jim Albano, Horizon BCBSNJ’s vice president of network management and Horizon Healthcare Innovations, said in a news release. “A growing number of our network doctors are embracing these initiatives and their collaboration has been vital to the success to date.”
