Geisinger Health System’s ProvenWellness Neighborhood has two “care pods” — one based at Geisinger-Mount Pleasant in Scranton, Pa., and the other at Wayne Memorial Community Health Center’s Honesdale (Pa.) Family Health Center. The model will focus on providing care to the uninsured population or under-insured with critical healthcare needs.
“Our ultimate goal is to overcome barriers to assuming quality healthcare for everyone,” said Thomas Graf, MD, CMO, population health and longitudinal care at Geisinger Health System.
The ProvenWellness Neighborhood aims at making patients more informed about their health and better able to navigate the healthcare system. In its first year, the care model is expected to serve more than 1,000 patients, and in its second year, it should serve roughly 17,000 patients.
Geisinger has been active in its experiments with payment and care delivery models. In May, the health system and xG Health Solutions, its affiliated data management company, announced partnerships with 20 additional hospitals for its Geisinger-xG Health bundled payment initiative. The news came ahead of the CMS’ expansion of the Bundled Payment for Care Improvement program.
