Cedars-Sinai, CareMore Health to coordinate care for seniors in LA

Los Angeles-based Cedars-Sinai and the Medicare Advantage CareMore Health Plan announced an agreement Wednesday to improve care coordination in order to keep patients healthy and out of the hospital.

Medicare Advantage patients enrolled in the CareMore Health Plan will have access to care from more than 60 primary care physicians and 130 specialists with Cedars-Sinai Medical Group and Cedars-Sinai Health Associates. Overtime, the panel of specialists will be expanded, the groups said in a news release.

Under the agreement, CareMore will open a Comprehensive Care Center near Cedars-Sinai to help patients manage chronic diseases, provide nutritional counseling, strength training and other services. CareMore physicians and social workers will also visit patients with limited mobility at home to help patients manage prescriptions and line up transportation for in-person visits to their physicians.
