A patient who was checking out of the hospital Nov. 22 received the paperwork for approximately 20 other patients in her discharge file, according to the report.

A nurse at the hospital allegedly took her file and removed the other patients’ records, but the patient said her discharge file still contained seven pages of operating room records, including patients’ birth dates, physician names, procedures and medications, according to the report.

The hospital launched an internal investigation and is notifying affected patients, according to the report. Additionally, the hospital is offering free identity theft protection to the affected patients for one year.
