Montefiore ACO (Bronx, N.Y.). Despite serving one of the most densely populated, low-income communities in the United States, Montefiore ACO is one of the more successful and progressive healthcare providers in the Pioneer ACO program. Montefiore generated 3.6 percent in gross savings and generated $8.43 million in shared savings in 2014. Montefiore ACO has participated in CMS’ Pioneer ACO program since 2012.
