Hospitals are building 2027 budgets while several of the biggest variables in those budgets remain unresolved.

States must implement Medicaid work requirements by Jan. 1. CMS projects the requirement will reduce Medicaid enrollment by about 2.3 million people in fiscal year 2027. Hospitals are still absorbing the effects of the enhanced ACA subsidies that expired at the end of 2025, and site-neutral payment and 340B policy continue to evolve. Premier estimates that changes to premium subsidies and Medicaid eligibility could reduce hospital revenue by more than $68 billion across 2026 and 2027.

For health system leaders, understanding the exposure is no longer the hard part. The harder question is when to act and when to wait while key policy questions remain open.

In this webinar, hospital and health system executives will share how they are adjusting financial assumptions and protecting strategic investments. They will also explain how they are using revenue cycle performance and technology to offset reimbursement pressure, and name the decisions they believe leaders must make before the year ends.

Learnings include:

  • How health systems are preparing for coverage disruptions tied to Medicaid work requirements, and which operational changes are proving most important
  • How payer behavior and rising denials are changing the financial picture, and how providers should respond
  • What role revenue cycle performance can play in offsetting reimbursement pressure
  • How leaders decide when to act and when to wait as site-neutral payment and 340B policy evolve
  • Where AI helps and where organizations still need human clinical and coding expertise
