The program is a partnership between Providence Hospitals and Right at Home, a nonmedical in-home assistance company funded by the Duke Endowment grant.
Right at Home provides a nonmedical worker to a recently released chronic care patient for four hours a day for four to six weeks. The worker assists the patient in daily tasks and helps the patient set up frameworks and strategies for caring effectively for him or herself.
Providence Hospital has a 22 percent readmissions rate, compared to the 25 percent national readmissions rate. Patients in the Right at Home program have a readmission rate of just 10 percent.
In addition to reducing readmissions, the partnership with Right at Home could also contribute to medical cost reductions, according to the news release.
