TeliStat earns $4.5M grant to reduce elderly hospital readmission in New York

The New York State Department of Health awarded a $4.5 million grant to TeliStat for its care model designed to reduce 30-day readmissions among elderly patients discharged from a hospital to a nursing home.

The TeliStat Restorative Care Unit Program works by designating a specific section of a skilled nursing facility to these at-risk patients. Within the unit, an integrated care team of multidisciplinary physicians and nurses care for patients using proprietary technology tailored to the care program. Patients are equipped with a cellular biometric monitor, which transmits data to the central nursing station.
