The Registered Nurse Ambulatory Care Coordinator Association will support care coordination and population health management by providing guidance and education. Later this year, RNACCa will offer a certification program in population health management through a partnership with Xavier University in Cincinnati.
RNACCa members will consist of nurse care coordinators and team members that support care coordination, such as social workers, patient-centered medical home coaches, case managers, registered dieticians, certified diabetic educators and pharmacists, according to the release.
