Researchers at the heart institute adapted the Intermountain Risk Score, which is used to predict mortality rates in trauma patients, and used a statistical model to create IMRS-HF. The tool combines information from a patient’s electronic medical record and other sources to calculate a score indicating patients’ likelihood of returning to the hospital within 30 days. The EMR would include the patient’s score to guide providers’ treatment decisions.
Salt Lake City-based Intermountain Healthcare plans to expand the tool to all its hospitals, according to the release.
