Connecticut Medical Society, ACP Launch Software to Help Physicians Become PCMH

The Connecticut State Medical Society-IPA and the American College of Physicians have launched online software for primary care practices interested in pursuing the patient centered medical home model of care.

The ACP developed Medical Home Builder 2.0 to help physician practices improve patient care, organization and workflow. MHB 2.0 is an Internet-based, easy-to-use software tool that will help assess physician practices’ readiness and capabilities as a National Committee for Quality Assurance-recognized PCMH.

CSMS-IPA is making the MHB 2.0 Tool available at no cost to eligible primary care practices participating in the IPA’s Patient Centered Practice Model program to help them improve their ability to provide more efficient, coordinated care.
