The ACP developed Medical Home Builder 2.0 to help physician practices improve patient care, organization and workflow. MHB 2.0 is an Internet-based, easy-to-use software tool that will help assess physician practices’ readiness and capabilities as a National Committee for Quality Assurance-recognized PCMH.
CSMS-IPA is making the MHB 2.0 Tool available at no cost to eligible primary care practices participating in the IPA’s Patient Centered Practice Model program to help them improve their ability to provide more efficient, coordinated care.
