Home Visits More Effective Than Calls in Managing Medications

Post-discharge home visits may be more effective in helping patients manage complex medication regimens than follow-up calls, according to research published in the Journal of Nursing Care Quality.

For this study, researchers compared two interventions during a pilot: follow-up telephones calls and a home visit by a registered nurse. The registered nurses delivered these interventions with the support of pharmacists.

 

Researchers found the nurses discovered 62 percent more medication discrepancies during home visits than telephone calls.
