Heart Failure Program Reduces Readmissions Down to 3%

A new heart failure program at Mary Lanning Memorial Healthcare in Hastings, Neb., helped dramatically reduce heart failure readmission rates, according to a KHASTV report.

Under the Health Enhancement Program, heart failure patients undergo intense education on how to prevent readmission and manage their health. The education takes places for four to six weeks after discharge and ranges from diet and physical activity to medication management.

 

The program was implemented at the healthcare organization in April 2011. Since then, heart failure readmission rates were reduced from 19.4 percent during the first quarters of 2011 to 3.7 percent.
