First “Health Home” Care Model for Patients With Chronic Disease to be Established in Missouri

Missouri will be the first in the nation to create a “health home” model for providing healthcare to Medicaid beneficiaries with severe and persistent mental illness or people with a mental health or substance use disorder coupled with either a second chronic condition or tobacco addiction, according to a CMS news release.
