Through the program, ARC staff will follow Medicare patients who experience a heart attack, pneumonia or congestive heart failure 30 days post-discharge. The staff members will help the patients understand the importance of medication management, follow-up physician visits and warning signs, according to the report. Eligible patients may also be provided home-cooked meals and transportation to physician visits.
The six hospitals that have partnered with ARC include Piedmont Hospital and Emory University Hospital Midtown in Atlanta, Southern Regional Medical Center in Riverdale, WellStar Cobb (Ga.), WellStar Kennestone in Marietta and Gwinnett Medical Center Lawrenceville (Ga.).
