M Health Fairview adopts patient hand off tool to improve safety

Minneapolis-based M Health Fairview is implementing a tool from I-PASS Patient Safety Institute to reduce communication breakdowns during patient handoffs.

The Joint Commission’s latest sentinel event report found such incidents increased 78 percent from 2020 to 2022, and communication breakdowns were a leading factor contributing to these incidents.

M Health Fairview aims to eliminate preventable harm by improving coordination among care teams with the universal handoff framework, the organizations said in a June 6 news release.
