The schedules were discovered missing Oct. 22, 2013. The schedules contained patients’ names, dates of birth, medical record numbers, dates of procedure, procedures, surgeon names and anesthesia types. While health system officials do not believe the information has been used inappropriately, the schedules have not been recovered and officials have chosen to notify affected patients.
