According to a June 14 news release from the Justice Department, Alfonzo Bailey operated Eye for Change Youth and Family Services in Cleveland, which offered mental health counseling, housing support and other services.
Mr. Bailey and his co-defendants defrauded Ohio’s Medicaid program by submitting false claims for mental health counseling. Mr. Bailey directed his staff to create false progress reports for services not actually rendered to patients, according to the Justice Department.
