Vermont HIE connects providers with home telemonitoring data

Vermont providers can now access patient vital signs from home telemonitoring devices, thanks to a collaboration on a statewide health information exchange.

The Visiting Nurse Association of Chittenden & Grand Isle Counties and Central Vermont Home Health & Hospice have connected their monitoring systems to their EHRs, with the help of a grant from the Vermont Health Care Innovation Project. The organizations also worked with Vermont Information Technology Leaders to transmit EHR data to the Vermont Health Information Exchange.

In the past, data from in-home devices could only be accessed within the telemonitoring system itself. If needed by an outside healthcare provider, the data would be manually transmitted through means such as a secure fax. Now, however, data collected at a patient’s home is immediately available to providers on the HIE — including 14 hospitals, 11 health centers and 155 other provider locations.
