The grant was awarded by the New York State Department of Health and funded by CMS. It was awarded as part of the Balancing Incentive Program’s $45 million Innovation Fund created to shift care into community settings.

St. Mary’s will use the funds to implement an interactive voice response program. Caregivers will receive short, automated calls and answer questions regarding their child’s condition, medication adherence and other information. The calls will generate reports and flag those that require outreach from providers, according to a statement from St. Mary’s.
