Skyward Health selects Foothold to perform IT maintenance across health home system

New York City-based health home system Skyward Health has selected Foothold Technology to oversee its IT operations.

Skyward Health cited Foothold’s platform, which handles both health home care management and billing, as a reason it was selected. Skyward Health serves 30,000 members across 15 New York counties, according to the Oct. 5 Foothold news release.

The health home model originated with the ACA to reduce preventable costs and involves care management agencies that help coordinate every aspect of some Medicaid members’ care, including medical care, housing and social services.

Foothold Technology will help Skyward navigate New York’s upcoming Medicaid payment and delivery changes.

“Skyward Health will be the champion that Medicaid members with chronic conditions deserve, one that simplifies an overly complex health system and advocates for every patient’s medical, emotional and social needs,” Skyward Executive Director Andrea Hopkins said. “Partnering with Foothold allows us to coordinate and analyze vast amounts of data and get the concise insights we need. Their experience has made it easy for our care coordinators to provide top-level service to our members.”
