UNC Health unveils rare disease coding tool in Epic

UNC Health, based in Chapel Hill, N.C., has launched what it called the world’s first standardized rare-disease coding tool, integrating Mondo Disease Ontology codes into its EHR.

As part of Epic’s February 2026 software update, health systems using IMO Core can now access Mondo disease codes directly within the Epic EHR, according to a Feb. 26 news release. UNC Health said the codes will be available across its 20 hospitals and more than 900 clinics within the year.

Mondo consolidates rare disease terminology from databases including ClinGen, Orphanet, the International Classification of Diseases and the Online Mendelian Inheritance in Man database into a single coding framework. The update introduces nearly 5,000 new rare disease codes and revises more than 25,000 related disease codes in Epic.

UNC leaders said the standardized coding aims to streamline diagnosis and improve care coordination for patients with rare diseases, many of whom do not have a specific ICD code.
