On Jan. 5, Trinity Health identified suspicious activity on an employee’s email account and launched an investigation into the incident.
The investigation determined that between Dec. 16 and Dec. 18 an unauthorized party had gained access to an employee’s email account that contained files with patient information.
Some of the patient information compromised due to the email breach include names, addresses, phone numbers, email addresses, prescription information, medical record numbers, patient ID numbers and dates of birth.
The report did not mention how many patients were affected.
On March 9, Trinity Health sent out notification letters to all affected individuals.
