The AHRQ will provide up to $1.5 million to cover the total costs of the project. The project must not exceed three years. The projects must demonstrate how scaling and spreading successful health IT-enabled practice models that use patient-reported outcomes measures improve outcomes.

“There is a need to scale and spread the successful use of PROs in a manner that is efficient, actionable by the physician, supports patients’ engagement with their health and care and improves clinical processes and outcomes,” the grants website states.

Applicants may submit their project proposals starting Jan. 25, 2017. The agency will accept applications on a rolling basis.
