Through the model, FFPC physicians can be rewarded through pay-for-performance structures if they meet targets for improving quality and lowering healthcare costs. Registered nurses will also serve as clinical care coordinators and help patients navigate the care continuum.
The program is believed to be the first patient-centered accountable care program in Northern Virginia between a family medical practice and health plan.
FFPC received level 3 certification as a patient-centered medical home from the National Center of Quality Assurance in 2011.
