The startup, called Cityblock, aims to improve health outcomes for low-income Americans in urban environments. The startup plans to connect each participant with a personalized care team comprised of physicians, coaches and technological tools.
“We believe health happens locally,” reads Cityblock’s website, which went live Oct. 2. “Cityblock is building a personalized health system that serves qualifying Medicaid or Medicare members who live in city neighborhoods that have traditionally gone without good health services.”
Cityblock aims to forge partnerships with insurance companies to ensure participants have access to these personalized services at no additional cost.
The startup plans to launch its services in early 2018.
