Partners in Care, a physician-owned provider network in East Brunswick, N.J., has partnered with Wellcentive for population health management and value-based care solutions.
Wellcentive will help Partners in Care track data and apply analytics to identify health trends and at-risk populations across its 700-strong provider base, and spanning 50,000-plus patients.

Wellcentive officials expressed equal excitement for the new partnership. "Partners in Care is a pioneer in population health management," said Wellcentive CEO Tom Zajac. "We are honored that an organization with this depth and vision selected us as their partner."
Wellcentive announced a services partnership with Portland, Ore.-based FamilyCare Health community care organization earlier this month.
