1. Nine healthcare professionals explain how U.S. healthcare costs have gotten so out of control over the past decade.

2. New York Downtown Hospital agreed to a $13.4 million settlement to resolve allegations of Medicaid and Medicare fraud.

3. Health Share of Oregon — the state's largest Medicaid coordinated care organization, which are part of Oregon's new Medicaid demonstration program — officially went live last Saturday.

4. Maine Attorney General William Schneider filed documents with the 1st Circuit Court of Appeals asking the court to review CMS' failure to decide on Maine's proposed Medicaid cuts.

5. Medicare Part B spending for noninvasive diagnostic imaging decreased 21 percent from 2006 to 2010, dropping from approximately $11.91 billion to $9.46 billion.
