naviHealth, a company that partners with health systems, accountable care organizations and post-acute providers to better coordinate care, has partnered with Golden Living, which has rehab, assisted living, skilled nursing, hospice and palliative care facilities in several states.
The partnership will use naviHealth's decision support technology, post-acute care analytics platform and clinician network to improve patient outcomes and transition of care.
Through the partnership, the two companies will share data, collaborate clinically on ER diversion and readmission programs and including appropriate Golden Living providers in any provider networks established by naviHealth.
