Blue Cross Blue Shield of Michigan Medical Home Program Triples in Size

Blue Cross Blue Shield of Michigan's patient-centered medical home program has nearly tripled in size since it launched in 2009, and the growth is punctuated by the payor's latest designation of 994 practices as PCMHs.

The 994 practices are "a record number" for the payor and a 28 percent increase from the number of PCMH-designated practices in 2011. This latest round includes 3,017 physicians. In 2009, about 1,200 physicians were designated.

Designation lasts for one year — from July through June — and practices have to earn designation each year. The payor said about 75 percent of practices designated in 2009 have remained designated each year since.

The payor also said 2012 data show its PCMH practices have patients requiring fewer hospital admissions and emergency room visits than those in non-PCMH settings.
