Humana, Iora Health partner for accountable care

Louisville, Ky.-based Humana and Brooklyn, N.Y.-based Iora Health will provide coordinated care for Humana's Medicare Advantage members at four new Iora Primary Care practices — two in Phoenix and two in Seattle.

Iora Health currently serves more than 9,000 patients at seven primary care practices. The four new practices, which are scheduled to start seeing Humana Medicare Advantage members on Oct. 1, will have care teams that include physicians, nurses, health coaches, medication management, behavioral health and social services resources.Humana Logo

The partnership between the two organizations includes value-based incentives tied to improving quality of care.

Humana has entered into many other accountable care agreements in recent months, including one with the Nebraska Health Network — an ACO co-owned by Omaha, Neb.-based Methodist Health System, The Nebraska Medical Center in Omaha, and their affiliated physicians.
