Effective for the 2018 plan year, Evolent will acquire Premier Health Plan’s Medicare Advantage and large group commercial assets.

Evolent partnered with Premier Health in 2013 and helped the health system launch its health plan. Under the acquisition, Evolent and the health plan will enter a long-term in-network agreement with Premier Health’s hospitals and physicians. Evolent also agreed to support Premier’s population health management initiatives under its employee, Next Generation ACO and payer partnerships.

The deal is expected to close in the first quarter of 2018.
