Pascale Carayon, PhD. Proctor & Gamble Bascon Professor in Total Quality, Department of Industrial and Systems Engineering at University of Wisconsin-Madison.

In addition to her role as a professor at the University of Wisconsin-Madison, Dr. Carayon directs the university’s Center for Quality and Productivity Improvement. She participated on the expert panel that reviewed the Institute of Medicine’s To Err Is Human report and contributed to the resulting Free from Harm patient safety report. She received the Human Factors and Ergonomics Society A.R. Lauer Safety Award in 2015 for her work developing a program to tackle problems related to medical error. She also received the 2015 John M. Eisenberg Patient Safety and Quality Award from the Joint Commission and the National Quality Forum for her use of human factors engineering and the Systems Engineering Initiative for Patient Safety model to advance the patient safety field.
