DeKalb Physician Hospital Organization (Decatur, Ga.). DeKalb Physician Hospital Organization incorporated in 1994 and has ACO contracts with several payers. In 2016, DeKalb PHO teamed up with UnitedHealthcare to create an accountable care program for more than 250,000 people with employer-sponsored UnitedHealthcare plans. DeKalb PHO also holds an ACO contract with Humana and participates in a collaborative care initiative with Cigna.
