The two hospitals are part of parent organization Lahey Health, based in Burlington, Mass.
The lab request forms contained patient names, health insurance identification numbers and some Social Security numbers, according to the report.
The forms were inside a zipped courier bag and were being transported for storage. The courier allegedly first stopped at Beverly Hospital’s health information management department to drop off other records. When he then arrived at Addison Gilbert Hospital, he could not find the lab request forms in the bag, according to the report.
Beverly Hospital has notified the patients of the misplaced forms and is retraining couriers and hospital staff, according to the report.
As of yet there is no indication the information has been used for fraudulent purposes, according to the report.
