The challenge called for innovators to develop digital solutions that could be used to connect providers and patients to health services addressing social determinants of health.
The finalists, chosen from a pool of more than 100 applicants, will next compete in a live pitch competition in September. The first place winner will receive $40,000, while the second and third place winners will receive $25,000 and $10,000, respectively.
The three finalists are:
- Community Resource Network (Grand Junction, Colo.): a social information exchange that provides information about social and environmental factors, genetics and individual behavior to those involved in a patient’s healthcare.
- Open City Labs (New York City): a platform that integrates with health information exchange systems to automatically connect eligible patients to community services and government benefits to address social determinants of health.
- Social Impact AI Lab (New York City): a group of social services agencies and technology providers using artificial intelligence technology to address social determinants in children.
