Teladoc's chronic care tools delivering improved health outcomes

Teladoc's approach to providing care to patients with multiple chronic health conditions led to better control of A1c, systolic blood pressure and weight better than any diabetes, hypertension or weight control program alone, the telehealth provider said July 15.

Teladoc's data found that people with both diabetes and elevated blood pressure, who used the company's chronic care integrated program, sustained clinically significant blood pressure reduction after only four weeks, according to a July 15 company news release.

Teladoc also found that the chronic care integrated program allowed:

  • People with diabetes and elevated blood pressure to lower their HbA1c, a clinical metric that reflects blood glucose control.

  • Mental health services to be integrated into chronic care management, which resulted in members seeing an average 0.5 percent additional A1c reduction.

  • Members to be more engaged while using multiple chronic condition management programs through a single, streamlined experience.
