Ascension Saint Thomas to launch in-home acute care services

Nashville, Tenn.-based Ascension Saint Thomas hospital partnered with Contessa, an acute home care management company, to offer its patients home recovery care services such as telehealth appointments and remote monitoring.

The services will roll out this fall for patients with acute, non-life threatening medical conditions such as congestive heart failure and pneumonia. A Saint Thomas physician examines the patient before he or she begins home care treatments to ensure the individual's safety. The patient also undergoes 30 days of remote monitoring after starting Contessa's services.

"The aim is to keep people healthy while understanding hospital care is sometimes needed," said Amber Sims, Ascension Saint Thomas chief strategy officer, according to a news release. "When hospital-level care is needed, we will offer the patient's home as a site of care to continue serving as the high quality, low cost health system offering an exceptional experience for our providers, patients and families."

Ascension Saint Thomas operates nine hospitals in Tennessee and as well as physician-led medical group Saint Thomas Medical Partners, which provides primary and specialty care services in Tennessee and Kentucky.
