Which States are Most and Least Healthy in the U.S.?

Vermont is the healthiest state in the country yet again, marking its sixth consecutive year in the top spot, according to a report from America's Health Rankings.

The rankings are based on data from several federal and reputable sources, such as the Centers for Disease Control and Prevention, the Federal Bureau of Investigation and the Dartmouth Atlas Project. The rankings are also based on a unique methodology, which focuses on human behaviors, environmental factors, healthcare policies and clinical care. The ranking is developed and annually reviewed by a committee of leading public health scholars.

Here are the rankings for all 50 states for 2012.
Scores indicate the weighted number of standard deviations a state is above or below the national norm.
