State health officials cited Lancaster (Pa.) Behavioral Health Hospital over two patient safety incidents that occurred earlier this year, reports WPMT FOX43.

A March 31 incident involved a patient who was supposed to have 24/7 supervision due to the risk of self-harm. An employee allowed the individual to close a curtain while using the bathroom, and the patient was found not breathing.

In a second incident, staff members gave a patient an incorrect medication dose. The medication error was flagged by the automated dispensing cabinet and when the medication was scanned for the patient, but staff members did not catch it. The error did not harm the patient.

The 126-bed behavioral hospital submitted detailed plans of correction for both incidents to the Pennsylvania Department of Health. Its license remains in good standing.
