A free toolkit on implementing patient safety rounds in healthcare organizations is available from the Dana-Farber Cancer Institute in Boston.

The patient safety rounds help providers identify and eliminate patient safety risks through two methods: Staff communicate with each other about actual and potential safety problems, and former patients and their family members interview current patients about their perception of safe care.
