The funds were granted to healthcare organizations willing to redesign patient care approaches to reduce avoidable hospital use and costs. Grants include $1 million to Nassau University Medical Center in East Meadow, N.Y.; Westchester Medical Center in Valhalla, N.Y.; Erie County Medical Center in Buffalo, N.Y.; and Finger Lakes Performing Providers System.

The funds follow a federal agreement in April for New York for reinvesting $8 billion in Medicaid savings to support hospital overhauls and expand primary care, according to the report.
