Northwell's home health agency recognized for heart failure care  

New Hyde Park, N.Y.-based Northwell Health's home health agency — Northwell Health at Home — received The Joint Commission's Gold Seal of Approval for Heart Failure Certification.

Three things to know:

1. The two-year designation was based on an on-site review the home health agency underwent in December 2018.

2. Northwell Health at Home's heart failure disease management program aims to reduce patients' symptoms, improve quality of life, and decrease hospitalizations and emergency room visits.

3. Northwell Health at Home features a team of about 2,000 nurses, therapists, social workers and home health aides who care for more than 50,000 patients annually.

"Ten percent of the patients we treat at home have a diagnosis of heart failure, which translates to about 5,000 patients a year," Merryl Siegel, senior vice president and regional executive director of Northwell Health at Home and post-acute services at Northwell, said in a press release emailed to Becker's. "The Joint Commission recognition demonstrates to patients and families that Northwell is a leader in heart failure care and uses the latest evidence-based guidelines to provide the highest level of care possible."
