Gartner, the leading American research and advisory firm publishes a population health management guide for healthcare providers every year. Titled “Market Guide for Healthcare Provider Population Health Management Solutions,” the guide scouts the healthcare market for vendor solutions and lists the ones that meet the baseline criteria of various surveys.
