UnitedHealthcare to expand use of digital data to personalize senior care

UnitedHealthcare plans to expand collection of digital data for Navigate4Me, a program that helps members enrolled in its Medicare Advantage plans manage chronic conditions.

Three things to know:

1. UnitedHealthcare launched Navigate4Me in 2017 to support members with chronic conditions such as diabetes or congestive heart failure. Nurses and customer service staff in the Navigate4Me program help these members navigate their care, from resolving claims or billing issues, to coordinating next steps in the member's care plan, to addressing social determinants of health.

2. To inform their care navigation strategies, Navigate4Me's staff uses health profiles developed through the UnitedHealthcare Nerve Center. The Nerve Center is a digital data hub that serves as a command center for the Navigate4Me program, centralizing data from various sources including claims, clinical systems and digital devices, such as blood pressure monitors, blood glucose monitors and wearables.

3. By reviewing a member's health profile, UnitedHealthcare navigators are armed with information about their routine checkups, emergency room visits and how well they are managing their condition at home. The intended goal of the program is to provide navigators with information to identify the member's upcoming care needs or risk of experiencing an adverse medical event.

"We're building the next generation of care management by moving away from traditional models to a holistic approach that harnesses the power of real-time data and technology to deliver the right kind of individual support that can help people get well and stay well," Brian Thompson, CEO of UnitedHealthcare Medicare & Retirement, said in a news release.
