Humana has a new value-based care payment program that centers on social determinants of health — and New Orleans-based Ochsner Health System is the model's first participant, the insurer said March 4.
Three things to know:
1. Humana's Social Determinants of Health Value-Based Program aims to improve health outcomes by focusing on four nonmedical factors that affect health: food insecurity, social isolation, loneliness and housing instability.
2. Under the program, Humana will pay Ochsner providers who better coordinate care for Humana members. Increased patient screenings and documentation of assessment findings, for example, will be considered in the program.
3. The Social Determinants of Health Value-Based Program is designed to work in concert with existing value-based agreements that Humana has with healthcare providers.
Philip Oravetz, MD, chief population health officer at Ochsner Health, said in a prepared statement that the "new program considers the impact of important factors that affect the well-being of our patients. It's an exciting development as we continue to adapt our approach to care to meet the comprehensive and diverse needs of the communities we serve."
